GPH Practice Data dictionary
Every plan includes every field. This page lists each field, what it means and how complete it is today.
Coverage percentages are filled by the build from the dataset itself and are pending.

| Linked organization | linked_org_id | Where several location profiles belong to one organization, the shared organization key. Empty when no link is held. | pending |
