Credentialing / Credentialing Status Check
How to use the optional credentialing tracker template
One case covers one provider, one insurance plan, one practice business and one service location for this application. Use another case when any of these change. Keep your own list of which provider each label represents.
Use the template; don't include names, IDs or patient details. Keep insurance company documents and the link between labels and real names in your own records. Choose a listed value in each category. Fill only dates you know; blank means unknown. You do not need to locate all 12 dates before preparing your application answers. Dates describe administration only. The template has no free-text note field. Do not add attachments or identifier columns. All dates must describe this same application.

Provider, plan, business and location checked means your practice checked that the records concern the same provider, insurance plan, practice business and location. It does not mean GetPracticeHelp checked the records.
A submitted application is different from a complete application acknowledged by the insurance company. Signed-contract receipt is different from the date you sent it. Keep both conflicting effective dates, if known. Choose only what your records confirm. Do not assume a later step is complete because an earlier step is complete. A requested date belongs in Requested date only; directory visibility belongs in Directory listing only.
